Social Work Services
Our Social Worker is on-site twice a week and is provided by Senior Transitions made simple.
Senior Transitions Made Simple (STMS) is a private social work practice servicing individuals living in the community and long-term care homes. Our vision is to create a better aging experience for seniors and their families. We do this by empowering seniors to make informed decisions and eliminate the overwhelm that comes with later life changes.
STMS is committed to ensuring social work services are delivered in abidance with the standards of professional practice and conduct of the Ontario College of Social Workers and Social Service Workers.
Our social work team supports residents and their families with their mental health and well-being, and helps those living with complex needs navigate the system. We consider the psychological, social, biological, spiritual, and financial factors that may affect the physical and mental health of residents and families.
Any staff member can connect you with social work or you can find one in the Wellness room.
